I authorize Springfield Baptist Church to photograph and/or video my child during Vacation Bible School. I understand that the photos and/or videos may be used on the church website or social media.
I, the parent/guardian of the participant listed above, acknowledge that participation in Vacation Bible School events may result in risks such as sickness or bodily injury. I release Springfield Baptist Church from any financial responsibility or legal action should sickness or injury occur.
I, the parent/guardian of the child listed above, authorize the members of Springfield Baptist Church to administer first aid treatment for any minor injuries my child may receive during Vacation Bible School. In the event of an emergency, I authorize Springfield Baptist Church to contact professional emergency personnel to attend, transport, and treat my child.
For safety reasons, we will need first and last names of all individuals who have permission to pick up your child from Vacation Bible School at Springfield Baptist Church. In the case of emergency or last minute change, please reach out to us so that we know who will be picking your child up.
Your child’s safety is our priority. If there is someone who does NOT have permission to pick up your child, please list the first and last name of the individual(s).
